Terms & Policies

RESERVATION POLICIES:

We currently do not ask for a deposit, although remain eligible to ask for a deposit at any time. Payments are taken after treatments. CB Sports Therapy & Rehabilitation accepts payment in cash, by cheque, bank transfer or through paypal

CANCELLATION POLICY:

If you are unable to make your appointment, please inform us 24 hours in advance via Facebook.

CB Sports Therapy & Rehabilitation has the right to cancel a treatment without notice if the room or environment is unsuitable for the treatment to be provided, or if the therapist is taken ill.

FACILITIES:

We bring all of our equipment with us, however all we ask is you have a safe, spacious environment/room for us to set up our massage table.

MEDICAL CONCERNS:

All clients must complete a consultation form prior to receiving Sports Therapy & Rehabilitation treatments. Prior to your treatment we will ask you about any medical concerns you have, it’s your responsibility to inform us of all medical issues that may concern your treatment.

PRIVACY:

Client treatment details will not be discussed with anyone other than the client. Consultations will only be discussed with the client. Consultation Forms and treatment notes will be kept on file, securely by CB Sports Therapy & Rehabilitation. By signing the consultation form, clients are giving CB Sports Therapy & Rehabilitation permission to hold records and data about them. Personal information will not be shared with any third parties.
